The short answer
To add someone to CHAMPVA, you file a new CHAMPVA application for that person, online or on VA Form 10-10d. Updating an existing application won't add them. Newborns need a Social Security number and to be added as the Veteran's dependent first. Adopted children need adoption papers, and stepchildren need proof of the Veteran's marriage to their parent.
Do you need a new application to add someone?
Yes. VA's after-you-apply page says adding a dependent is one of the times you file a new CHAMPVA application.
That's different from updates. Changes to your address, insurance, or school status go on your existing application. A new person needs their own application.
You can apply online or on paper with VA Form 10-10d.
What does each new family member need?
VA's CHAMPVA page lists these documents:

If you're filing on paper and the new person has other insurance or Medicare, also include VA Form 10-7959c. That form reports insurance. It doesn't add anyone by itself.
How do you add a newborn?
In this order:
- Get the baby's Social Security number.
- Add the baby as the Veteran's dependent with VA.
- File a new CHAMPVA application for the baby as soon as you can.
VA can't pay the baby's medical claims until the baby is enrolled. Keep every bill from the birth and the first weeks. Once VA sets the baby's effective date, you or the providers can file claims for covered care on or after that date. More here: CHAMPVA for children under 18.
Does adding someone guarantee coverage?
No. The new person still has to meet CHAMPVA's rules, including not being eligible for TRICARE under 38 CFR 17.271. A marriage, birth, or adoption doesn't turn on CHAMPVA by itself.

Common mistakes
- Calling to "add" someone without filing an application. It doesn't work. File the application.
- Sending only Form 10-7959c. That's an insurance form, not an application.
- Skipping the newborn dependency step. Do it before the CHAMPVA application.
- Leaving out adoption papers or the stepchild's marriage proof. Those are required.
What happens next?
If VA approves the new person, it mails a welcome letter and a CHAMPVA ID card. If VA asks for something, add it to that application instead of starting over. More here: After you apply for CHAMPVA.
