The short answer
It depends on the drug and the diagnosis. VA lists Mounjaro, Ozempic, Trulicity, and Victoza for type 2 diabetes; Zepbound for moderate to severe sleep apnea; and Wegovy for MASH or preventing heart attack and stroke after prior events. Wegovy and Zepbound need prior authorization through OptumRx. No GLP-1 is covered for weight loss alone.

Prior authorization for Wegovy and Zepbound goes to OptumRx at 844-403-1029. That's separate from CHAMPVA's medical preauthorization line.
This is a fast-changing area. Check VA's page before relying on coverage.
Why isn't weight loss covered?
38 CFR 17.272(a)(22) excludes nonsurgical weight-loss treatment, including prescription drugs. So the same drug can be covered for one diagnosis and not another. The prescription has to be for the listed condition.

What about Rybelsus?
VA's pages don't match. VA's Meds by Mail list includes Rybelsus for type 2 diabetes, but the CHAMPVA provider page doesn't list it. Ask the pharmacy or prescriber to confirm before relying on payment.
Zepbound for sleep apnea
The sleep apnea has to be moderate to severe and confirmed by a sleep study within the past 12 months, and prior authorization is required. If your sleep study is older, you may need a new one.
Wegovy for MASH or heart risk
Prior authorization is required. VA describes the heart use as prevention for people who've already had a major cardiovascular event. The prescriber should send documentation of the diagnosis and answer any follow-up questions from OptumRx.
If a GLP-1 prescription is rejected
- Check the diagnosis on the prescription. Does it match VA's list?
- Check prior authorization for Wegovy or Zepbound.
- Check the sleep study date for Zepbound.
- Separate pharmacy problems from coverage decisions. Routing, other insurance, and missing approval are different fixes.
- Get the exact rejection reason before trying again.
Does Meds by Mail change anything?
No. Meds by Mail is a way to get the drug, not a way around the coverage rules. You also need no other drug coverage to use it. More here: CHAMPVA prescription coverage.
