Which CHAMPVA number should I call?
For most questions, call the VA Customer Call Center at 800-733-8387, Monday through Friday, 8:05 a.m. to 7:30 p.m. Eastern, according to VA's CHAMPVA provider page. You can also write through Ask VA. Start here when you are not sure which office you need. Providers new to CHAMPVA use this line too.
CHAMPVA uses different numbers and addresses for different tasks. Sending something to the wrong place slows it down, so match the contact to the job.

How do providers verify eligibility?
By phone at 888-820-1756, 24 hours a day, with the beneficiary's Social Security number and the provider's tax ID ready. Or electronically with a 270 eligibility request through a clearinghouse using payer ID VAHAC. For claim payer IDs, see CHAMPVA ID Card and Payer Information.
What are the pharmacy contacts?
Retail pharmacies
Pharmacists can reach the OptumRx help desk at 888-546-5503. Pharmacy claims use BIN 610593, PCN VA, Group HAC. Beneficiaries with retail pharmacy questions can call 888-546-5502.
Meds by Mail

Which CHAMPVA forms will I need?
VA keeps them on its CHAMPVA forms page. The main ones:
- VA Form 10-10d: Application for CHAMPVA benefits.
- VA Form 10-7959A: Beneficiary claim form. Not for providers.
- VA Form 10-7959C: Other health insurance certification, at enrollment or when your coverage changes.
- VA Form 10-7959D: Potential liability claim, used when another party may be responsible, such as after an accident.
- School Enrollment Certification Letter: For dependent children ages 18 to 23 staying on CHAMPVA as full-time students.
- VA Form 20-0995 and VA Form 20-0996: Supplemental Claim and Higher-Level Review.
Before you mail or fax anything
- Make sure the address matches your task. Eligibility, beneficiary claims, provider claims, and appeals all go to different places.
- Download a fresh copy of the form from VA.
- Keep a copy of everything.
- Send sensitive information only through official VA channels.
